Payment Information

We are committed to providing you with quality medical care. Your clear understanding of our financial policy is important to our professional relationship.

We only bill and collect fees for the facility, Saint Francis GI Endoscopy, LLC. The fees for your physician, anesthesia, pathology and laboratory fees (if any specimens are obtained) are separate and not part of SFGIE fees.

 

Prior to your procedure, assignment of benefits and confirmation of coverage are obtained from your insurance provider.  Please contact us prior to your procedure at (860) 683-9991 Ext. 104, for co-pay and deductible questions.

Insurance

 

Insurance is a contract between you and your insurance company.  SFGIE is not a party to your contract.  We will not become involved in disputes between you and your insurance company regarding deductibles, non-covered charges, co-insurance payments, secondary insurance and coordination of benefits or pre-existing conditions other than to provide factual information as necessary.

 

The patient is responsible for deductibles and coinsurance payments and co-pays.  Any medical care determined by your insurance company to be a “non-covered benefit” is also your responsibility.  It is very important that you notify us of any changes in your insurance coverage.  Incorrect information could mean delays in payment from your insurance company.

Medicare

 

SFGIE is a licensed Medicare Provider.  We will bill Medicare directly for fees associated with your care.  As a beneficiary of Medicare, you are responsible for deductibles and co-insurance.

Private Pay

 

If you are paying with cash or credit card, please make arrangements in advance.  Payments and/or deductibles are due at time of services.
